GENERAL & REFERRALS
Do I need a referral to make an appointment?
No. You can book an appointment directly with Diabetes Wellness Coaching without a referral.
However, you may be eligible for a Medicare rebate if your GP refers you under a GP Chronic Condition Management Plan. Speak with your GP before your appointment if you would like to explore this option.
Do you offer Medicare rebates?
Yes. Medicare rebates are available for eligible appointments when you have a valid referral from your GP under a GP Chronic Condition Management Plan.
Private health insurance rebates may also be available depending on your provider and level of cover.
Do you offer Telehealth appointments?
Yes. Telehealth appointments are available and can be a convenient option for diabetes reviews, insulin pump and CGM support, troubleshooting, and ongoing education.
Where are appointments available?
Face-to-face appointments are available in Melbourne CBD and Preston, with Telehealth also available for people across Australia.
INSULIN PUMP & TECHNOLOGY SUPPORT
Do you support all insulin pumps?
Yes. Diabetes Wellness Coaching provides independent education and support across insulin pump and automated insulin delivery systems.
The aim is to help you understand the available options and choose a system that best suits your diabetes, lifestyle and preferences.
Can you help me choose an insulin pump?
Absolutely. If you’re considering insulin pump therapy, an appointment can be used to discuss the different systems available, including their features, advantages and limitations. The aim is to give you the information you need to make an informed decision about which pump works best for you.
There is no expectation that you already know which pump you want before making an appointment.
Can you help me start a new insulin pump?
Yes. Insulin pump starts and education are a major focus of Diabetes Wellness Coaching.
Support can include preparation before your pump start, initial settings, carbohydrate ratios and correction factors, hands-on training, automated insulin delivery education, troubleshooting and follow-up after commencement.
Can you help if I’m already using an insulin pump?
Yes. You don’t need to be starting a new pump to book an appointment.
Existing pump users can receive support with settings optimisation, glucose patterns, exercise, hypoglycaemia, hyperglycaemia, carbohydrate bolusing, automated insulin delivery and troubleshooting.
Do you provide CGM support?
Yes. Support is available for continuous glucose monitoring systems, including sensor education, interpretation of glucose reports, alerts, troubleshooting and using CGM data to make informed diabetes management decisions.
Can you review my pump or CGM reports?
Yes. Reviewing your glucose and insulin data can help identify patterns that aren’t always obvious day to day.
Where appropriate, we can review your reports together and discuss potential adjustments to insulin delivery, carbohydrate ratios, correction factors, glucose targets and diabetes management strategies.
PATIENT GROUPS & SPECIFIC CARE
Do you see children and adolescents?
Yes. Diabetes Wellness Coaching provides diabetes education and technology support across paediatric and adult diabetes care.
Support can also include parents, caregivers and other people involved in a young person’s diabetes management.
Can you help with exercise and type 1 diabetes?
Yes. Exercise and automated insulin delivery are particular areas of interest.
Appointments can explore insulin adjustments, carbohydrate strategies, exercise or activity modes, insulin-on-board and strategies for reducing both hypoglycaemia and post-exercise glucose excursions.
Do you only see people with type 1 diabetes?
No. While insulin pumps, CGM and type 1 diabetes are key areas of focus, diabetes education can be provided for people living with different types of diabetes depending on their individual needs.
I’ve just been diagnosed with diabetes. Can I book an appointment?
Absolutely. A new diagnosis can involve a huge amount of information in a very short period of time.
Appointments can focus on the practical aspects of diabetes management at a pace that works for you, including insulin, glucose monitoring, carbohydrate counting, hypoglycaemia, exercise, sick-day management and diabetes technology.
ATTENDANCE & BOOKING LOGISTICS
Can my partner, parent or caregiver attend?
Of course. You’re welcome to bring a partner, family member, parent or caregiver to your appointment.
Diabetes affects much more than the person wearing the device, and having the people around you understand how to provide support can be incredibly valuable.
What should I bring to my appointment?
If relevant, bring your glucose meter, insulin pump, CGM receiver or phone, insulin and any diabetes-related information you would like reviewed.
If you use an insulin pump or CGM, having your device connected to its reporting platform before your appointment can also help us make the most of our time together.
How do I book?
Appointments can be booked online for face-to-face or Telehealth consultations.
